A form created for the Rhode Island census mandated in 1782 by An act for granting and apportioning a tax of twelve thousand pounds, in silver or gold, upon the inhabitants of this state (Evans 17698); with a column for the names of heads of households, followed by columns for eight gender and age categories under each ethnic heading, and a final column for the total per family; recto and verso have the same form.
